Liberty Hospital will host a free opportunity May 2 to complete legal forms in recognition of National Healthcare Decision Day. Notaries will be available to help participants complete a durable power of attorney for healthcare form. A notarized durable power of attorney for healthcare form is a legal document that expresses an individual’s healthcare wishes when he/she is unable to do so.
In this document, an individual appoints a person to serve as his/her primary healthcare agent to make any necessary care or treatment decisions. This person then is responsible for communicating those wishes to your attending physicians and care providers. This is a free community service open to the public. Notaries and members of the Liberty Hospital Risk Management team will be available from 7:00 a.m. to 6:00 p.m. in the lobby at the main entrance of Liberty Hospital, 2525 Glenn Hendren Drive, Liberty. No appointments are necessary.
